CASE MANAGEMENT
Professional nurse and social work case managers provide FREE care coordination services for families who have children/youth with special health care needs. We partner with families to open doors to the best health care and community resources specialists, equipment/supplies, community programs; family support and health insurance. We teach families about the special education process. Care Connection for Children is a program through VDH and is operated in Northern Virginia by the Inova Health System.
